Non-active duty, prior service Reserve Component (National Guard and Army Reserve) SSG and below, and DOD civilians. Soldiers holding MOS 91A, 91K, 91P, 91V, and 91WM6 will not be considered. Must have Academy of Health Sciences Certificate of Eligibility for completion of Distance Learning 513-91G10(RC)(DL) Phase 1, before the unit can make a reservation for the resident course. The service remaining requirement (SRR) upon completion of this course is governed by NGR 351-1 (1 year) and AR 135-200 (1 year). Prior to the departure from home station, soldiers must reenlist or extend their terms of enlistment to fulfill the SRR upon course completion; those not meeting the SRR when reporting to resident training will not be accepted into the course unless they reenlist or extend. USAR Soldiers are TEMPORARILY waived the requirement to fulfill the Ready Reserve obligation upon course completion, effective for the duration of Partial Mobilization. Enlisted women who are pregnant must be processed IAW AR 635-200. The Armed Services Vocational Aptitude Battery (ASVAB) test minimum scores required in aptitude area CL are: 95 for tests administered prior to 2 January 2002; 92 for tests on and after 2 January 2002 and prior to 1 July 2004; 90 for tests on and after 1 July 2004. No history of a felony conviction or conviction of crimes involving: weapons, ammunition, explosives/arson charges; drug activity involving illegal possession, buying, selling or distribution of controlled substances or synthetics; violence against persons or property; sexual misconduct. No record of conviction by special or general courts-martial or civilian courts of offenses listed in Chapter 24 of AR 27-10 (Military Justice), or otherwise required to register as a sexual offender under chapter 24, AR 27-10. The physical profile serials (PULHES) apply to initial entry soldiers only, and are not to be used as a prerequisite for soldiers reclassifying into this MOS. |
