This is Phase 1 (Distance Learning [Distributive Training] ) of a Two-Phased Course. Phase 2 is an ATRRS-based two week Resident Training. Reserve Component Soldies will arrive at Fort Sam Houston with the following items: MPRJ 201 file, Medical Records, Dental Records and complete basic clothing issue, to include running shoes. National Guard and Army Reserve SSG's will receive a certificate of completion for attending this course, additional skill level 30 training is required to be awarded the MOS. SUBMIT ALL WAIVER REQUESTS TO: CDR, AMEDDC&S, AMEDD Personnel Proponent Directorate, MCCS-DE, Fort Sam Houston, TX 78234-5052.


Prerequisite
For the resident phase: Reserve Component (National Guard and Army Reserve) SSG and below. Soldiers holding MOS 91A, 91K,91P,91V and 91WM6 will not be considered. Must have Academy of Health Sciences Certificate of Eligibility for successful completion of 551-91J10 RECLASS (DL) Phase 1. The service remaining requirement (SRR) upon completion of this course is governed by NGR 351-1 (1 year ) or AR 135-200 (1 year). Enlisted women who are pregnant will be processed IAW AR 635-200. Prior to the departure from home station, soldiers are required to re-enlist or extend their terms of enlistment in order to fulfill the SRR upon completion of the course. When reporting to the AMEDDC&s for training, and it is determined that soldiers do not meet the SRR, they will not be accepted into the course unless the SRR is fulfilled. The physician profile serials (PULHES) applies to initial entry soldiers only and is not to be used as a prerequisite for soldiers reclassifying into this MOS. A minimum score of 95 in aptitute area CL in Armed Services Vocational aptitute Battery (ASVAB) tests administered prior to 2 January 2002. A minimum score of 92 in aptitute area CL on ASVAB tests administered on and after 2 January 2002 and prior to 1 July 2004. A minimum score of 90 in aptitute area ST on ASVAB tests administered on and after 1 July 2004
